Patients living within the practice area are welcome to register, you are eligable to register if you have a CM8 postcode. In this respect please bring along proof of address such as a recent utility bill. We also require photographic identification such as a drivers licence, please be aware drivers licence is not a valid form of proof of address. To register you we also need your NHS number, which you will be able to get from your previous GP. If you are on any regular medication we require proof in the form of a prescription or a repeat slip.

A registration form (GMS1) is available from reception, which you will need to complete. We also ask new patients to complete a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Children are not required to complete a health questionnaire. 

Medical treatment is available from the date your registration is approved. Approval and confirmation from the NHS usually takes approximately 2 working days.  Please contact reception for further information.

It is a requirement of NHS England that every patient is allocated an accountable GP. This does not mean that you can only see your accountable GP you may book an appointment with the GP of your choice subject to an appointment being available.

If you are over the age of 16 we will need you to personally bring in your registration form along with you photographic ID, and will not accept registration forms that have been bought in on someone else behalf.
