New Patient Registration
Patients living within the practice area are welcome to register, you are eligable to register if you have a CM8 postcode. In this respect please bring along proof of address such as a recent utility bill / bank statement. We also require photographic identification such as a drivers licence, please be aware drivers licence is not a valid form of proof of address.
To register you we also need your NHS number, which you will be able to get from your previous GP. If you are on any regular medication we require proof in the form of a prescription or a repeat slip, also you will need to ensure that you have at least two weeks supply of your medication during the registration process.
Medical treatment is available from the date your registration is approved. Approval and confirmation from the NHS usually takes approximately 2 working days.
It is a requirement of NHS England that every patient is allocated an accountable GP. This does not mean that you can only see your accountable GP you may book an appointment with the GP of your choice subject to an appointment being available.
If you are over the age of 16 we will need you to personally bring in your registration form along with you photographic ID, and will not accept registration forms that have been bought in on someone else behalf.
